Common list of things to bring or send for tax returns:

  • W2s, 1099s, any other income
  • Form 1095—Proof of health insurance 
  • If you have an HSA (Health Savings Account) we’ll need forms 1099-SA which shows the distributions and form 5498-SA which shows the contributions
  • If you own your home:  mortgage statement showing interest paid and property tax paid
  • If you rent:  Name and address of landlord and amount paid for year
  • Any income interest (from banks, etc)
  • Copies of current drivers licenses 
  • 401K or IRA statements 
  • Amounts of any donations made during the year 
  • Student loan interest 
  • If claiming children: Proof of residency for children (ex: school transcript, doctors records, etc)
  • Daycare provider name, address, tax ID # and amount paid for the year
  • For direct deposit of any refund into checking or savings account: bank name, account number, routing number, and whether it is checking or savings
